Complete the required UCF Immunization Health History Form and submit it through the Med+Proctor portal along with your immunization records (if required). All students must provide proof of two MMR (measles, mumps, and rubella) vaccinations. Hepatitis B and meningitis vaccinations may be waived by completing the waiver section of the health history form.
